You are responsible for utility costs for your property or unit starting on the official Move In Date. (This date can be found on your lease.) When setting up your utilities you will provide a start date: make sure your start date is the day your lease begins. Utilities must be transferred into the tenants name two weeks prior to Move In.

This form gives proof that utilities have been transferred. Please submit only 1 form per rental unit.

If this form is not submitted 2 weeks prior to Move-In you may not be granted access to your rental unit.
